Developing Transitional Partnerships
The overall aim of the SACCS Recovery Programme is to prepare each child to live once again in a substitute family living environment.
With this in mind SACCS are committed to supporting a child's transition back to the placing local authority wherever possible and when it’s in the child’s best interest. In recognition of the need to have the ability to recruit, train and support national transitional placements SACCS have began to explore and develop working partnerships with regional and nationally recognised and approved Foster Agencies. However, for those children who are deemed not able to return to the placing local authority SACCS Care will work in partnership with our sister company, Flying Colours Foster Care, to identify, train and support a more local provision as an option.
Transitional Planning
During the later stages of the Recovery programme (normally after the 4th review of the Individual Recovery Programme) SACCS Care will be working in close liaison with the Local Authority, Flying Colours and or a National Foster agency partner to identify an appropriate family placement for the child.
Once identified SACCS Care will propose a carefully stepped transitional plan for the child from residential to a family placement. This transition typically takes four months although this will be determined by the individual needs of each child.
As part of the transitional planning current and future levels of ongoing individual and placement support will be considered and a plan will be drawn up identifying the responsibility, frequency, and duration of the support.
